In the ancient city of Perugia, a chilling fear weaves itself into the fabric of everyday life as a faceless serial killer stealthily preys on unsuspecting college students. The relentless Inspector Martino is the beacon of hope in this dark time, armed with the ominous clue of a black and red scarf, a sinister signature left by the elusive murderer. Street vendor Gianni Tomasso, a purveyor of scarves, becomes entangled in this deadly game of cat and mouse. His inability to recall who purchased the scarf leads him down a perilous path, transforming him from the questioned to the victim. As the investigation deepens, the town's terror escalates, with everyone wondering who the next victim might be. Amidst this gripping horror, a group of art history students yearn for a haven from their studies and the growing dread. American Jane, along with her companions Daniela, Katia, and Ursula, retreat to the secluded sanctuary of Daniela's uncle's villa, hoping to find solace in the beauty of the countryside. However, unbeknownst to them, they are carrying the shadow of the killer with them. When Jane injures her ankle, the suave Dr. Roberto is summoned to the villa, introducing a new character to their secluded haven. Meanwhile, the obsessive Stefano Vanzi lurks in the background, his fixation on Daniela adding another eerie layer to the unfolding mystery.
