Kang Cheol, once a renowned brawler ruling the streets of Busan, has traded his fists for a life of tranquility, devoting himself to the care of his ailing mother. Life takes a pleasant turn as he embarks on a tender romance with Su-ji in a serene seaside town. However, the peaceful interlude is shattered when the sinister underbelly of Busan pulls him back. Cheol stumbles upon Sang-gon and Hwi-gon, notorious gangsters, cunningly camouflaging a Yakuza's demise as an accident. His past comes back to haunt him when an old acquaintance, Jong-su, betrays him to the ruthless gang.
In the face of his mother's escalating medical bills, Sang-gon presents Cheol with a tempting offer: to plunge back into his old life and work for the gang. Despite the pressing need, Cheol's honour resists, but a relentless Jong-su delivers a crippling blow by offering Cheol's home as loan collateral to Sang-gon. Stripped of options, Cheol finds himself at a crossroads, forced to reconsider his pledge to lead an honorable life.
